As another health and safety measure, Parents/Guardians will be dropping off medication directly to the school nurse by appointment. Parents/Guardians are asked to complete this formif they have medication that will need to be administered, or housed in school. Upon completion, the nurse at your child's school will call to arrange a date and time for you to come to the building to drop off the medication. Please note that all medication must be delivered by the parent/guardian in the original container, accompanied by the original, signed physician/dentist authorization form. Moving forward, drop off appointments will be scheduled on Wednesdays.
